Mattress Firmness, Explained: How to Choose the Right Level for Your Body
Firmness is the most important mattress characteristic and the one consumers get wrong most often. The mattress industry does not help. There is no universal firmness standard, no regulatory definition, and no consistent scale across brands. A "medium firm" from one manufacturer may feel like a "firm" from another, and a mattress that feels perfect in a showroom after two minutes of lying down may feel wrong after a full night of sleep. The result is a $20 billion industry with a return rate that hovers around 15 percent — one in seven mattresses goes back because the buyer chose the wrong firmness.

What Firmness Actually Measures
Firmness describes how much resistance a mattress offers when weight is applied. Mattresses at 1 to 3 are rare outside of specialty therapeutic applications, and mattresses at 9 to 10 are essentially rigid surfaces that few people would choose to sleep on.
Firmness is not the same as support. This is the distinction that trips up most mattress shoppers. Support refers to the mattress's ability to maintain spinal alignment by pushing back against the body's heavier sections (hips and shoulders) while conforming to its lighter sections (waist and neck). A mattress can be soft and supportive (high-density foam that contours closely while maintaining alignment) or firm and unsupportive (a rigid surface that holds the body on top without conforming to its curves, creating pressure points and gaps). The ideal mattress provides the firmness level you find comfortable while delivering the support your body requires for neutral spinal alignment.
Sleep Position: The Primary Determinant
Your preferred sleep position is the single most important factor in choosing mattress firmness. The reason is geometric: different sleep positions place different demands on the mattress to maintain neutral spinal alignment — the position in which your spine maintains its natural S-curve without lateral bending, rotation, or excessive flexion.
Side sleepers need a softer mattress (4 to 6 on the firmness scale). When sleeping on your side, the shoulder and hip bear the majority of your body weight on a narrow surface area. A mattress that is too firm pushes back against these pressure points, causing them to bear weight without sinking in. This creates pressure-point pain in the shoulder and hip and forces the spine to bend laterally — the waist drops between the two support points, and the lumbar spine curves downward. A softer mattress allows the shoulder and hip to sink in proportionally, bringing the spine into a straight, neutral line from head to pelvis.
Back sleepers need a medium to medium-firm mattress (5 to 7). Sleeping on the back distributes weight more evenly across the full length of the body, reducing the need for pressure-point contouring. The primary concern for back sleepers is lumbar support: the natural curve of the lower back creates a gap between the body and the mattress, and if the mattress does not fill this gap with gentle upward pressure, the lumbar spine flattens, which can produce lower back stiffness and pain. A medium-firm mattress provides enough pushback to maintain the lumbar curve while contouring enough to prevent pressure under the shoulders and heels.
Stomach sleepers need a firmer mattress (6 to 8). Sleeping on the stomach places the heaviest part of the body (the pelvis) directly on the mattress surface. If the mattress is too soft, the pelvis sinks deeper than the chest and legs, creating hyperextension of the lumbar spine — essentially an exaggerated arch in the lower back that compresses the posterior spinal structures. A firmer mattress prevents this by keeping the pelvis from sinking excessively, maintaining a flatter, more neutral spinal position.
Combination sleepers who change positions frequently throughout the night generally do best with a medium firmness (5 to 6) that provides acceptable performance in all positions without being optimized for any single one. The trade-off is that a combination sleeper's mattress is a compromise — a side sleeper who also spends time on their back will not get the same shoulder contouring as a dedicated side sleeper on a softer mattress, but they will avoid the spinal misalignment that a too-soft mattress would cause during their back-sleeping periods.
Body Weight: The Modifier
Body weight modifies the firmness recommendation because heavier bodies exert more force on the mattress surface, causing deeper compression. A mattress that provides adequate support for a 130-pound side sleeper may bottom out under a 230-pound side sleeper, losing its ability to contour proportionally and creating pressure points and alignment problems.
The general adjustment: add one firmness point for every 50 pounds above 150 pounds. A side sleeper at 150 pounds does well on a 5 (medium). At 200 pounds, a 6 (medium firm) provides similar performance. At 250 pounds, a 7 (firm) is typically necessary. For people above 250 pounds, mattress construction becomes as important as firmness. Standard all-foam mattresses may lack the deep-compression support needed to maintain spinal alignment under higher loads. Hybrid mattresses (foam comfort layers over a pocketed coil support core) generally perform better for heavier sleepers because the coil system provides consistent pushback even at deeper compression depths, while the foam layers provide the surface-level contouring.

What the Research Says
The largest and most methodologically rigorous study on mattress firmness and back pain was published by Kovacs and colleagues in The Lancet in 2003. In a randomized, double-blind trial of 313 adults with chronic low back pain, participants slept on either a medium-firm or a firm mattress for 90 days. The medium-firm group reported significantly less back pain, reduced disability, and better daytime functioning compared to the firm group. This study directly contradicted the long-standing clinical recommendation to sleep on a "firm" mattress for back pain and remains the most frequently cited evidence in the field.
A follow-up study by Jacobson and colleagues published in the Journal of Chiropractic Medicine (2010) found similar results: replacing old mattresses with new medium-firm mattresses reduced back pain, shoulder pain, and spinal stiffness while improving sleep quality in a cohort of adults with and without preexisting pain conditions. The improvement was observed regardless of the firmness of the participants' previous mattresses, suggesting that the benefit was partly due to the new mattress's fresh support structure and partly due to the medium-firm firmness being closer to optimal for most body types.
The evidence converges on a clear recommendation: for the general population, medium to medium-firm (5 to 7 on the 10-point scale) is the safest choice. It provides adequate support for most body types and sleep positions while minimizing pressure-point pain. Deviations from this range should be based on specific body characteristics (sleep position, weight, existing pain conditions), not on subjective impressions from a brief showroom trial.
How Body Weight Affects Firmness Perception
Mattress firmness is not an absolute property — it is a relationship between the sleeper's body weight and the mattress's resistance to compression. A mattress rated as "medium firm" by a 140-pound side sleeper may feel distinctly firm to them but register as "medium" or even "medium soft" to a 220-pound sleeper, because the heavier person compresses the comfort layers more deeply and engages the support core sooner. This is why firmness ratings on manufacturer websites, which typically reflect testing by a 150-to-170-pound reference sleeper, can be misleading for people outside that weight range.
For sleepers under 130 pounds, softer mattresses (3-5 on a 10-point firmness scale) generally provide better pressure relief because lighter bodies do not compress foam deeply enough to access the contouring layers of firmer models. A mattress that feels "medium" to an average-weight tester may feel like sleeping on a board to a 115-pound side sleeper, particularly in the shoulder and hip zones where pressure concentration is highest. Conversely, sleepers over 230 pounds typically need a firmness rating of 7 or higher to prevent excessive sinking that causes spinal misalignment. A mattress that sags under heavy loads creates a hammock effect where the hips drop below the shoulders, straining the lumbar spine and often causing lower back pain that develops gradually over weeks rather than appearing immediately.
Couples with significant weight differences face a particular challenge. A 130-pound partner and a 210-pound partner will perceive the same mattress as two entirely different firmness levels, sometimes separated by two or more points on a 10-point scale. Split-firmness mattresses, where each side of the bed uses a different comfort layer, solve this problem directly. Alternatively, a medium-firm mattress (6-7 on the scale) paired with a softer topper on the lighter partner's side achieves a similar result at lower cost.
How Sleeping Position Changes Firmness Requirements
Your primary sleeping position is the single most important variable in firmness selection because it determines which parts of the body bear the most pressure and where spinal alignment is most vulnerable. Side sleepers need softer surfaces — typically in the 4 to 6 range on a 10-point firmness scale — because the shoulder and hip must sink into the comfort layer to maintain a neutral spine. A mattress that is too firm for a side sleeper creates pressure points at the shoulder and hip that cause the sleeper to shift positions repeatedly throughout the night, fragmenting sleep architecture and reducing time spent in restorative deep sleep.
Back sleepers require moderate firmness in the 5 to 7 range, with enough give to support the lumbar curve but enough resistance to prevent the pelvis from sinking below the shoulders. Stomach sleepers need the firmest surfaces, 6 to 8 on the scale, because a soft mattress allows the pelvis to drop and hyperextends the lumbar spine — the primary cause of morning lower back pain in prone sleepers. If you split time between two positions, prioritize firmness for the position in which you spend the most time. Sleep tracking data, if available, provides a more accurate picture of your actual position distribution than your subjective sense of how you sleep, which research shows tends to overestimate time in the preferred position by 20 to 30 percent.
Common Buying Mistakes
Testing for too short a period. Two minutes on a showroom mattress tells you almost nothing about how the mattress will perform over eight hours. The surface feel during the first minute is dominated by the comfort layer, which compresses quickly and feels the same across a wide range of support structures. The support characteristics that determine spinal alignment only become apparent after several minutes of sustained pressure. If you are shopping in a store, lie on each candidate for a minimum of 10 minutes in your primary sleep position. If shopping online, choose a brand with a 100-night or longer trial period and actually use the mattress for at least 30 nights before deciding — your body needs two to four weeks to adapt to a new firmness level.
Equating firmness with quality. A harder mattress is not a better mattress. The perception that "firm is good for your back" persists despite direct evidence to the contrary. Unless you are a stomach sleeper or weigh more than 230 pounds, you almost certainly do not need a firm mattress, and choosing one based on the belief that it is "better for you" is one of the most common causes of mattress returns.
Ignoring the adjustment period. A new mattress that feels different from your old one is not necessarily wrong. If your previous mattress was eight years old and sagging, your body has adapted to sleeping on an unsupportive surface, and a properly supportive new mattress will feel unfamiliar. Give it 30 nights. If the mattress is still uncomfortable after a full month, the firmness is likely wrong. If discomfort resolves within two to three weeks, it was an adjustment period, and the mattress is probably right.
Not considering the foundation. A mattress performs differently on different foundations. A foam mattress on a slatted bed frame with widely spaced slats (more than 3 inches apart) will sag between the slats, creating an uneven surface that distorts the intended firmness. Platform beds, bunkie boards, and slatted frames with closely spaced slats (2.5 inches or less) provide a consistent, flat surface that allows the mattress to perform as designed.
